I am training for mountain hiking and as you may know, going down a steep hill with a backpack is not as easy as it sounds. Its actually harder on the knees that going uphill, so I want to train for it. My treadmill will incline up, but not down. If I (or an electrician) could modify it to make it run backwards, I could train for downhill hiking. Is that feasible? Could I have a switch to change the direction from forwards to backwards?

You could always incline the treadmill yourself with things under it.
Not really easy to do. What you CAN do, however, very easily is buy a couple of concrete blocks and place them under the back feet, maybe with a 2×4 between them, and tip the unit forward (down) that way.
the wiring is designed to make the motor turn one way on these units today….you would have a very hard time making it go the other way…
The treadmill will probably use a synchronous AC motor, which can run only in one direction. Using an complex controller and reversing the commutator it is possible, but not really feasible…
Do what the others said and pile a bunch of stuff under one end of the treadmill.
